Remarks
On January 21, 2015, the Issuer and the reporting person amended and restated the original convertible note to extend its maturity date by one year and reduce the conversion price. This is being reported as a disposition to the issuer of the original convertible note and an acquisition from the issuer of the amended and restated convertible note.
