- F1On May 15, 2017, the reporting person gifted 110 shares from the Bennion Revocable Living Trust dated 12/19/2002. This gift is being voluntarily reported on this Form 4.
- F2Since the reporting person's last report on Form 4 dated March 13, 2017, he has transferred 673 shares directly owned by him to the Bennion Revocable Living Trust dated 12/19/2002.
