- F1On October 30, 2017, the Issuer effected a 1-for-4 reverse stock split of its common stock (the "Reverse Stock Split"). The amount of securities reported on this Form 4 have been adjusted to reflect the Reverse Stock Split.
- F2The restricted common stock granted shall vest and become exercisable in 1/36th increments each month over a three year period commencing on the first month from the date of grant.
