- F1The options were granted pursuant to the Issuer's 2011 Equity Incentive Plan in connection with the Issuer's initial public offering, which closed on February 14, 2011. The original Form 4 filed on February 15, 2011 is amended by this Form 4 amendment to report this transaction, which was inadvertently omitted from such original Form 4, but which has described in the Issuer's prospectus filed pursuant to Rule 424(b)(4) under the Securities Act of 1933, as amended, on February 10, 2011.
