- F1This Form 5/A is being filed to report a gift of 339,686 shares from the Michael A. Weiss Trust Agreement Gamma #3 to Mr. Weiss' living trust. Amounts previously reported as beneficially owned directly by Mr. Weiss in column 5 of the original Form 5 filed on March 13, 2015 have been corrected accordingly.
- F2This trust is a grantor retained annuity trust for the benefit of Mr. Weiss and his descendants. Mr. Weiss is not the trustee of the trust and disclaims beneficial ownership of these securities except to the extent of his pecuniary interest therein.
