Jason R. & Dana M. Nunn, trustees of the Jake & Dana Nunn Living Trust dtd 7/7/06 (the "Nunn Living Trust") received 4,271 shares of Common Stock of the Issuer in the distribution by NEA Partners 12 on November 9, 2015.
