Remarks
The Form 4 to which this amendment relates contained incorrect CIK and CCC codes, which mistakenly caused "L. Patrick Giroir, Jr." to appear as the name of the reporting person in the filed report on EDGAR instead of "L. Patrick Gage." No such transaction by Mr. Giroir involving Tetraphase Pharmaceuticals, Inc. derivative securities occurred. The Form 4 should be disregarded. A separate Form 4 under the correct CIK code for Mr. Gage has been filed.
