- F1Restricted stock units convert into common stock on a one-for-one basis.
- F2On September 9, 2016, the reporting person was granted 3,000 restricted stock units, vesting in four equal installments beginning on the first anniversary of the grant date. The reporting person resigned from the issuer's Board of Directors effective September 11, 2017, and accordingly the 2,250 unvested restricted stock units held by the reporting person at the time of his resignation were forfeited pursuant to the terms of the agreement governing the award. Amounts presented have been adjusted to reflect the one-for-five reverse stock split effected on February 28, 2017.
