- F1As previously reported, on July 29, 2015 the reporting person received a grant of 5,000 shares of performance based restricted stock. A portion of the performance metrics were met, therefore a portion of these restricted shares vested on the third anniversary of the date of grant and the remaining shares were automatically forfeited.
